The Questionnaires toolbars

The Questionnaires module includes the Questionnaires toolbar, the Dentrix modules toolbar, and the Patient Information toolbar.

Questionnaires

Button

Name

Description

Refresh

Click to refresh the Questionnaires module after you make changes.

Print

Click to open the Print Questionnaire dialog box.

Select Provider/Staff

Click to open the Select Provider dialog box from which you can select a provider or staff member.

View Responses for All Patients

Click to view all questionnaire responses from all patients for a selected period of time. This is the default view when you open Questionnaires from another Dentrix module in which you don't have a patient selected, such as the Office Manager.

Update Patient Information

Click to open the Update Patient Information dialog box from which you can update the patient’s record in the Family File to match new Questionnaire information.

Note: This button is only active when a form that has responses is selected in the Questionnaires list box.

Enter New Responses

Click to open the Select Questionnaire Form dialog box where you can select a questionnaire form to enter responses into.

Edit Responses

Click to edit the responses for the selected questionnaire.

Move Responses

Click to move responses from one patient to another.

Delete Responses

Click to delete the responses for the selected questionnaire.

Questionnaire Signatures

Click to open the Questionnaire Signatures dialog box from which you can sign the questionnaire and enter signature information.

List All

Click to list all questionnaires in the selected tab.

View Mode

Click to open the Questionnaire View window from which you can view the questionnaire form in a larger view, see the changes that have been made to the questionnaire form since the previous responses, and compare responses to previous responses for the same form.

View Questionnaire

Click to toggle the Questionnaire View pane on and off.

Dentrix Modules

You can use the Dentrix modules toolbar to open other Dentrix modules.

Patient Chart

Enter patient treatment and conditions, document and track patient care, enter periodontal treatment information, and explain and demonstrate proposed treatment to patients.

Family File

Store and display important patient information, such as the patient’s name, address, phone number, medical alerts, birth date, insurance coverage, employer, and referral information.

Ledger

Enter payments, file claims, print statements, set up financial agreements, and more.

Appointment Book

Schedule and manage your patient appointments electronically.

Office Manager

Use several tools and reports to set up, evaluate, and improve the management processes of your practice.

DentriXlink

Provides links to supported third-party X-ray and imaging systems you've installed on your computer; if none are installed, clicking this button displays a list of third-party digital X-ray and imaging systems that you can use with Dentrix.

Quick Letters

Combine data from your Dentrix database with the word processing abilities of Microsoft Word to create a letter for a single patient.

Quick Labels

Print a variety of labels for the selected patient.

Continuing Care

Monitor your patients’ pending dental care and communicate information and reminders easily and effectively with groups of patients.

Send Message

Create and send email messages to patients in your Dentrix database.

Health History

Create a record of a selected patient's medical conditions, allergies, medications, and prescriptions. The icon is white if you haven't assigned a medical condition, allergy, medication, or prescription to the selected patient.

Prescriptions

Create prescriptions and accurately track medicines prescribed to your patients.

Office Journal

Keep a record of past events and set reminders of future events.

Document Center

Make your office paperless by scanning, capturing, and importing many types of documents, such as patient letters, EOBs (explanation of benefits), and referral letters or images (such as patient pictures and X-rays). You can attache these documents to patients, providers, insurance carriers, and referral sources.

Treatment Manager

Access vital patient information, including treatment-planned work, balance, payment information, Office Journal entries, insurance information, and so forth.

Lab Case Manager

Linked to the Appointment Book so you can create a case for the patient, ship it to the lab, and track its progress.

eDex

Manage and quickly search lists of patients (active, inactive, or archived), employers, insurance carriers, labs, non-patients, providers, staff, referrals, and other contact you may want to remember. You can also create custom contacts and contact categories to filter the Contacts List or even locate a patient's next appointment.

Dentrix Marketplace

Open the Dentrix Marketplace website from which you can purchase many products to enhance and expand your Dentrix platform.

Patient Information

You can use the Patient Information toolbar to see information associated with your patients in the Dentrix database.

Medical Alerts

Click to display any medical alerts for the selected patient.

Patient Alerts

Click to display any patient alerts for the selected patient.

Patient Picture

Click to open the Patient Picture window, where you can save and edit a full-colour patient picture that you can access from the Appointment Book, Family File, Patient Chart, Treatment Planner, Treatment Manager, Document Center, and other Dentrix modules.

Patient Referrals

Click to display any patient referrals.

More Information

Click to open the More Information dialog box for the selected patient.
