Patient survey
Patient Survey replaces the new patient forms, health history update, and other forms that you use to collect information from patients. You can use Patient Survey to create a custom set of questions that you can print for your patients to answer.

Once patients complete the questionnaire, you can enter their answers into Easy Dental and store them digitally without having to use paper charts. Best of all, when you need to update the information, you can create a new copy with the previous answers, and your patient only has to modify those items that have changed since the last visit.
