There can be a circumstance when you need to print a blank ADA claim form.
Note: Generating a blank ADA claim form in Office Manager requires the "Reports" security right. Viewing or printing a blank ADA claim form in the Batch Processor requires the "Batch Processor, Print/Display Reports" security right.
To generate a blank ADA claim form
In Office Manager, on the Reports menu, click Blank ADA Form.
Note: This option is available only if your user account has been granted the "Reports" security right.
The Dentrix Reports dialog box appears.
Select the option that corresponds to the claim form that you want to generate, such as 2019 Claim Form.
If the selected claim form option is the one that you usually print, to save your claim form selection for the next time you print a blank ADA claim form, select the Save As Default Claim Form checkbox.
Click Yes to send the blank ADA claim form to the Batch Processor.
Preview and then print the blank ADA claim form, or print the blank ADA claim form without previewing it.
