Linking conditions to procedures
You can attach up to 20 conditions (diagnostic codes) to a procedure. Only the conditions that have been charted for a patient previously are available to be attached to any of that patient's procedures. Also, a condition can be linked to multiple procedures.

Conditions (diagnoses) can be associated automatically with a procedure when charting a quick exam. Chart one or more conditions; and then, with the same tooth (or teeth) selected, treatment plan a procedure to treat those conditions.

When you complete a procedure with linked conditions, Dentrix Ascend prompts you to choose which, if any, of the associated conditions you want to mark as Treated.

When you create a pre-authorization for procedures with linked conditions, the corresponding ICD-10 codes are attached to the pre-authorization.

When you create a claim for procedures with linked conditions, the corresponding ICD-10 codes are attached to the claim.

You can remove diagnosis codes from a treatment-planned or completed procedure before creating a pre-authorization or claim for the procedure, and you can remove ICD-10 codes from pre-authorizations and claims as needed.

If the correct patient is not already selected, use the Patient Search box to access the patient's record.
Note: You can include inactive patients in the search results by setting the Include inactive patients switch to On.

On the Patient menu, under Clinical, click (or tap) Progress Notes.
The patient's clinical record opens with the Progress Notes tab selected.
On the Progress Notes tab of a patient's clinical record, select a procedure.

Do one of the following:

On the Patient menu, under General, click (or tap) Overview. The patient's Overview page opens. Click (or tap) the Ledger box.

On the Patient menu, under Financial, click (or tap) Ledger.
The patient's Ledger page opens.

On a patient's Ledger page, select a procedure.

The Edit Procedure dialog box appears.

Select the Diagnosis tab.

In the Diagnostic Codes search box, begin typing part of a condition's ICD-10 code, SNOMED code, or description to search for a condition that has been charted for the patient. Continue typing as needed to narrow down the results list, and then select the correct diagnosis. Repeat this process as needed to attach other diagnoses to the procedure.

Click (or tap) Save.
