Charting conditions
Since the Chart is a clinical record of patient care, it must be completely accurate. To maintain an accurate clinical record, you can chart a patient's conditions.
To chart a condition
1. Select a patient.
2. If necessary, change the patient's provider.
Note: By default, all treatments posted in the Chart are assigned to the patient’s primary provider.
3. If the treatment requires a tooth number, click the appropriate tooth in the graphic chart.
4. Click the Conditions procedure category button.
The Select Procedure Code dialog box appears.
5. Select the appropriate condition from the list, and then click OK.
Note: If the condition requires additional information, such as surfaces or quadrants, the Surface Selection dialog box appears. Select the appropriate options, click Add, and then click OK.
6. Click the status button appropriate to the condition that you are charting (Complete, TX Plan, or Existing).
The condition is posted to progress notes with a status of CON, and if applicable, marks the selected tooth with a symbol appropriate to the condition in the graphic chart.
