On the Statement Forms tabĀ of the Preferences dialog box, you can set up billing statement preferences. All billing statement forms that are available for Dentrix Enterprise are listed.
To set up billing statement preferences
Do any of the following:
To change the description of a form, select that form, edit the name in the Descriptive Name box, and then click Change.
Select the checkboxes of the forms that you want to have available for selection as the default billing statement in the Practice Defaults.
Clear the checkboxes of the forms that you want to exclude from the Practice Defaults.
With a form selected, to move that form to the top of the list, click To Top.
Click OK.
