In Chart, you can add a patient's laboratory orders (for example, blood tests, biopsies, and cultures) to which you can attach results.
Notes:
You cannot use this feature for items such as crowns and prosthetics.
Accessing a patient's lab orders to add an order requires the "Manage Lab Tests" security right.
To add a lab order
With a patient selected in Chart, do one of the following:
Click the Orders button on the toolbar.
On the File menu, click Orders.
If applicable, do one of the following:
If the Password - Manage Lab Tests dialog box appears, user verification has been assigned to the "Manage Lab Tests" task. Do one of the following:
If your user account has been granted the "Manage Lab Tests" security right, enter your credentials in the User ID and Password boxes, and then click OK.
If your user account has not been granted the "Manage Lab Tests" security right, have a user with permission temporarily override this restriction so you can manage lab orders this one time. The overriding user enters his or her credentials in the User ID and Password boxes and then clicks OK.
If a message appears, informing you that you do not have rights to this operation, your user account has not been granted the "Manage Lab Tests" security right. To have a user with permission temporarily override this restriction so you can manage lab orders this one time, do the following:
On the message, click Yes.
In the Password - Manage Lab Tests dialog box, the overriding user enters his or her credentials in the User ID and Password boxes and then clicks OK.
The Orders dialog box appears.
Under Orders, click Add Order.
The Add Lab Order dialog box appears.
Set up the following options:
Order Date - The date that the lab test was ordered. By default, today's date is selected, but you can specify a different date. To change the date, do one of the following:
Enter the correct date (in a m/d/yyyy format) in the box.
To select a date, do the following:
Click the calendar icon to view a month calendar.
Navigate to the correct month using the left and right arrows.
Note: To quickly change the year and month, click the month-year at the top of the calendar, navigate to the correct year using the left and right arrows, and then click the correct month.
Click the correct day.
Provider - The provider who ordered the lab test. By default, the patient's preferred provider (Prov1 in Family File) is selected, but you can specify a different provider. To change the provider, do the following:
Click the search button .
The Select Provider dialog box appears.
Either leave the ID and Last Name boxes empty to get a list of all providers, or enter all or part of a provider's ID or last name in the ID or Last Name box, respectively, to search for a provider.
Under Search By, select one of the following options: This Clinic, to search for the provider in the clinic that you are currently logged in to; All Clinics, to search for the provider in all clinics; or My Clinics, to search for the provider in the clinics to which you have been granted the "My Clinics, Select Provider " security right.
Notes:
The All Clinics option is available only if your user account has been granted the "View All Providers" security right for the Central clinic and if your user account has not been granted the "My Clinics, Select Provider" security right for the clinic that you are currently logged in to.
The My Clinics option is available only if your user account has been granted the "My Clinics, Select Provider" security right for at least one clinic and if your user account has not been granted the "View All Providers" security right for the Central clinic.
Either select the All Providers option to include providers who are associated with clinics (and who have a Log On User ID), or select the In ApptBook option to include providers who have been set up in Appointment Book.
Click the search button .
The matching primary and secondary providers appear in the list.
Select the correct provider.
Click OK.
Scheduled Test Date - The date that the lab test is scheduled to be started. To specify the date, do one of the following:
Enter the correct date (in a m/d/yyyy format) in the box.
To select a date, do the following:
Click the calendar icon to view a month calendar.
Navigate to the correct month using the left and right arrows.
Note: To quickly change the year and month, click the month-year at the top of the calendar, navigate to the correct year using the left and right arrows, and then click the correct month.
Click the correct day.
Description/Panel - A description of the lab order or panel.
Lab Test - A lab test that is being ordered. To select a lab test, do the following:
Click Select.
The Select Lab Test dialog box appears.
Note: The lab tests that are stored in your database will be available when you click Search.
Do one of the following:
To get a list of all lab tests, do not enter any search criteria.
To search for lab tests by name and/or ID, enter the first character or more of a lab test name and/or ID in the Enter Test Name and Enter ID boxes, respectively.
Click Search to view a list of lab tests.
If no search criteria is entered, on the message that appears, asking if you want to list all lab tests, click OK.
Select the correct lab test.
Click OK.
The Test Name and Test ID of the selected test appear for your reference.
Note: To clear the lab test, click Clear.
Order Status - The status of the lab test. Select one of the following options: Ordered, In Progress, Completed, or Canceled.
Note: A lab test with a status of Ordered or In Progress can appear in a "Plan of Care" section of an exported C-CDA file for a clinical summary or transition of care document.
Lab Information - The laboratory where the lab test is being or was sent. To select a lab, do the following:
Click the search button .
The Select Lab dialog box appears.
If the list is long, to filter it by lab name, in the Enter Lab Name box, begin entering the name of a lab. As you type, the first lab that matches is selected automatically.
Note: If the correct lab is not listed, you can add the lab's information. You can edit or delete a lab's information only if that lab was added manually.
In the list, select the correct lab or Unspecified (to not specify a lab).
Click OK.
Additional Information - Notes regarding the lab test.
Click OK.
You are returned to the Orders dialog box.
Click Close.
