In Chart, you can delete a condition or diagnosis that was posted incorrectly.
Notes:
To view a condition or diagnosis before deleting it, the "Conditions/Diagnosis, Edit" security right is required. Then, to delete the condition or diagnosis, the "Conditions/Diagnosis, Delete" security right is required.
To quickly delete one or more conditions and/or diagnoses by clicking a button, the "Conditions/Diagnosis, Delete" security right is required.
To delete conditions and/or diagnoses
With a patient selected in Chart, on the Progress Notes panel, do any of the following as needed:
To view and then delete a condition or diagnosis, do the following:
Double-click a condition or diagnosis.
Note: If the condition or diagnosis that you want to delete has been posted but is not listed on the Progress Notes panel, verify that the correct status view option has been selected for the progress notes.
If applicable, do one of the following:
If the Password - Conditions/Diagnosis, Edit dialog box appears, user verification has been assigned to the "Conditions/Diagnosis, Edit" task. Do one of the following:
If your user account has been granted the "Conditions/Diagnosis, Edit" security right, enter your credentials in the User ID and Password boxes, and then click OK.
If your user account has not been granted the "Conditions/Diagnosis, Edit" security right, have a user with permission temporarily override this restriction so you can edit a condition or diagnosis this one time. The overriding user enters his or her credentials in the User ID and Password boxes and then clicks OK.
If a message appears, informing you that you do not have rights to this operation, your user account has not been granted the "Conditions/Diagnosis, Edit" security right. To have a user with permission temporarily override this restriction so you can edit a condition or diagnosis this one time, do the following:
On the message, click Yes.
In the Password - Conditions/Diagnosis, Edit dialog box, the overriding user enters his or her credentials in the User ID and Password boxes and then clicks OK.
The Change Condition dialog box appears.
Click Delete.
If applicable, do one of the following:
If the Password - Conditions/Diagnosis, Delete dialog box appears, user verification has been assigned to the "Conditions/Diagnosis, Delete" task. Do one of the following:

Click OK.
To quickly delete one or more conditions and/or diagnoses by clicking a button, do the following:
Select one or more conditions and/or diagnoses.
Notes:
If the conditions and/or diagnoses that you want to delete have been posted but are not listed on the Progress Notes panel, verify that the correct status view option has been selected for the progress notes.
It is possible to also select existing, existing-other, treatment-planned, and completed procedures, but only the selected conditions, diagnoses, existing procedures, existing-other procedures, and treatment-planned procedures will be deleted.
To select multiple conditions and/or diagnoses so you can delete them simultaneously, click a condition or diagnosis, and then, while pressing the Ctrl key, click the other desired conditions and/or diagnoses. To select a range of adjacent conditions and/or diagnoses, click the first condition or diagnosis of the desired range of conditions and diagnoses, and then, while pressing the Shift key, click the last condition or diagnosis of the desired range of conditions and diagnoses.
Click the Delete button .
A confirmation message appears.

If your user account has not been granted the "Existing Procedures, Delete" security right, have a user with permission temporarily override this restriction so you can delete existing procedures this one time. If you are also deleting planned treatment:
If the Password - Treatment Plan, Delete dialog box appears, user verification has been assigned to the "Treatment Plan, Delete" task. Do one of the following:
If your user account has been granted the "Treatment Plan, Delete" security right, enter your credentials in the User ID and Password boxes, and then click OK.
If your user account has not been granted the "Treatment Plan, Delete" security right, have a user with permission temporarily override this restriction so you can delete planned procedures this one time. The overriding user enters his or her credentials in the User ID and Password boxes and then clicks OK.
If a message appears, informing you that you do not have rights to this operation, your user account has not been granted the "Treatment Plan, Delete" security right. To have a user with permission temporarily override this restriction so you can delete planned procedures this one time, do the following:
On the message, click Yes.
In the Password - Treatment Plan, Delete dialog box, the overriding user enters his or her credentials in the User ID and Password boxes and then clicks OK.
If any procedures are selected and are not treatment-planned, existing, or existing-other, a message appears, informing you that only the conditions, diagnoses, existing procedures, existing-other procedures, and planned procedures will be deleted. Click OK.
A confirmation message appears.
Click OK.
