Adding consent forms

In the Dentrix Presenter you can add and digitally sign informed consent forms to a case presentation.

1.   If the Informed Consent Form(s) tab does not appear in the Case Presentation panel, in the toolbar, click Add Case Presentation Item, and then click Informed Consent Form(s) from the menu.

2.   Click the Informed Consent Form(s) tab in the Case Presentation panel.

The Dentrix Presenter dialog box with the forms selected for the case appears where you can create and add the consent form.

3.   Do the following:

·        Have the patient read and then sign the consent form in the Patient/Representative Signature text box.

·        Have the practice representative or doctor sign the consent form in the Practice Signature text box.

Note: You can sign the consent form using a light pen, mouse, or electronic signature pad.

4.   Click Save.

Note: Once you save a consent form, it is stored in the Treatment Planner Support Information. You cannot edit or delete a signed and saved consent form.
