Dentrix consent forms are electronic and can be signed electronically. By default, these forms include Consent for Anesthesia, Consent for Treatment, and Decline Treatment Consent. The forms are stored in the Treatment Planner Supporting Information panel. You can edit these forms and add new ones in the Settings section of the Treatment Planner.
To create a consent form
1. To create a consent form, click the Settings tab, scroll to Setup Consent Forms, and then click New.
The Edit Consent Form dialog box appears.
2. Type the name of the new consent form in the <New Consent Form> box, and then type the text of the consent form in the Note box.
Note: You can enter up to 5,000 characters of text in the consent form. Click the following buttons to edit the form's text: Cut, Copy, Paste, Insert Date Line, Check Spelling, and Display Font Size.
3. If you do not want to include procedure codes on the consent forms, select Do not include Procedure Codes on Consent Form.
4. To save the form, click Save, and then click Close.
