You can create customized forms within Dentrix Questionnaires to collect
information from your patients.

Additional Information
When designing a form, use a Patient Information Template when the
answers to that question will tie back directly to a place in the
patient's Family File or Health History. The patient's current
information will auto-populate the forms, and if the patient makes
changes to that information, the answers will easily update into the
patient's file in Dentrix.
When inserting Note questions, the option "Use Note to Update Patient
Information" will appear. If selected, the answer the patient provides
can later be used to create or append any of the following notes:
