Printing patient visit forms
The Patient Visit Form is a route slip that you can customize to view or print for a certain patient or multiple patients. You can filter the forms that are generated by primary provider, appointment provider, operatory, and appointment date. The form can include patient, family, account, recall, and insurance information; patient and family alerts; patient and account notes; treatment-planned procedures; and future appointments.
To print a patient visit form
1. Select the appointment that you want to print a Patient Visit Form for.
2. In the Schedule toolbar, click the Print button, and then click Print Patient Visit Form.
The Patient Visit Form dialog box appears.
3. Set up the following options:
· Select Patient – Indicates the patients that you want to generate visit forms for:
· Patient for Selected Appointment – Generates the form for the patient of the currently selected appointment. This option is not available if you generate the form from Reports or if you generate the form from the Schedule and do not have an appointment selected.
· All Patients – Generates a form for any patient who meets the other criteria that determine if Easy Dental generates a form.
· Other Patient – Generates the form for a different patient (other than the patient of the currently selected appointment). Click the search button to select the appropriate patient.
· Select Primary Provider – Generates forms for patients by their primary providers. Click the From and the To search buttons to select the starting and ending provider, respectively, or leave <ALL> selected in both fields to include all primary providers. To specify one provider, select that provider for From and To. These options are not available if you selected Patient for Selected Appointment. For a new patient appointment (an appointment with an “NP” next to the patient name), Easy Dental uses the appointment provider as the primary provider for the form.
· Select Appointment Provider – Generates forms for patients who are scheduled to see certain providers only. Click the From and the To search buttons to select the starting and ending appointment provider, respectively, or leave <ALL> selected in both fields to include all appointment providers. To specify one appointment provider, select that provider for From and To. If you selected Patient for Selected Appointment and then select an appointment provider or range of providers that does not include the provider attached to the currently selected appointment, the form will be blank.
· Select Operatory – Generates forms for patients who have appointments in certain operatories only, select a range of operatories. Click the From and the To search buttons to select the starting and ending operatory, respectively, or leave <ALL> selected in both fields to include all operatories. To specify one operatory, select that operatory for From and To. If you selected Patient for Selected Appointment and then select an operatory or range of operatories that does not include the operatory for the appointment currently selected, the form will be blank.
· Appointment Date – Generates forms for patients who have appointments within a certain date range only. To select a range of dates, click the From and the To search buttons to select the starting and ending date, respectively. The date range cannot be open-ended; you must have a valid date for both From and To. By default, the date range is the current system date or, if you selected an appointment in the Schedule, the date of the currently selected appointment. To specify a single date, select that date for From and To. These options are not available if you selected Patient for Selected Appointment.
4. Select or clear the following options:
· Include Detailed Patient Information – A section on the form with information from the Patients module and the Patient Banner: contact numbers, address, and primary and secondary providers.
· Include Patient's Recall – A section on the form with the Recall types assigned to the patient.
· Include Patient Alerts – A section on the form with the patient’s alerts.
· Include Family Alerts – A section on the form with the alerts for the patient’s family.
· Include Patient Notes – A section on the form with the patient’s notes. To restrict the note to the first line, select First Line Only.
· Include Referral Information – A section on the form with the patient’s referral information.
· Include Account Information – A section on the form with the patient’s account information, such as the guarantor’s name, billing type, last insurance payment details, last guarantor payment details, date the last statement was sent, and family balance.
· Include Guarantor Account Note – A section on the form with the account notes for the patient’s family. To restrict the note to the first line, select First Line Only.
· Include Treatment Plan Procedures – A section on the form with the patient’s treatment-planned procedures. Select one of the following to indicate which procedures to show on the form:
· Scheduled for this Appointment – The treatment-planned procedures attached to the patient’s appointment only.
· All for Patient – All treatment-planned procedures for the patient.
· Include Insurance Information – A section on the form with the patient’s insurance information. Select any of the following options:
· Primary Dental – The patient’s primary dental insurance information:
· Detailed – Basic carrier and subscriber details, plus benefits and coverage details.
· Brief – Basic carrier and subscriber details.
· Secondary Dental – The patient’s secondary dental insurance information:
· Detailed – Basic carrier and subscriber details, plus benefits and coverage details.
· Brief – Basic carrier and subscriber details.
· Include Family Information – A section on the form with Recall information for each of the patient’s family members (excluding those with an “Archived” status). And then, select the three Recall types that you want in the Family Information section from the following lists:
· Select First Recall To Include For Family –
· Select Second Recall To Include For Family –
· Select Third Recall To Include For Family –
· Include Patient's Future Appointments – A section on the form with the appointment information for each of the patient’s future appointments.
5. Select Do Not Close When Preview/Print/Batch if you want the Patient Visit Form dialog box to remain open when you preview, print, or batch the form.
6. Select Save As Default to save the currently selected report options, except Select Patient and Select Primary Provider, for the next time you generate the form.
7. Do one or more of the following:
· Preview – Click to preview the forms before printing.
· Send To Batch – Click to send the forms to the Batch Processor to be viewed or printed at a later time.
· Print – Click to send the forms to the default printer.
8. If you selected Do Not Close When Preview/Print/Batch when you generated the last form, and you have finished generating forms, click Close.
