Changing diagnostic codes

Since the Chart is a clinical record of patient care, it must be completely accurate. To maintain an accurate clinical record, you can correct errors in the Chart to reflect the actual care given by changing the diagnostic codes you may have erroneously attached to a procedure.

Important: If you have already created an insurance claim for the diagnostic code that you want to change, you must delete the insurance claim in the Accounts module.

To change a diagnostic code

1.   In the progress notes list, double-click the procedure that you want to change that has a diagnostic code attached.

Note: Procedures with a diagnostic code attached have the letter “D” in the Description text box.

The Edit or Delete Procedure dialog box appears.

2.   Click the Diagnoses search button.

The Select Diagnostic Code(s) dialog box appears.

3.   Select the diagnostic code that you want to change in the Selected Diagnostic Codes list, and then click Remove or Clear Selected Diagnostic Codes.

Note: To select more than one diagnostic code, click each code that you want remove or clear.

4.   In the list on the left of the dialog box, select the diagnostic code that you want to attach instead of the code you cleared, and then click Add.

5.   Click OK twice.

The diagnostic code is changed in the progress notes.
