Starting a clinical note review (Evaluator)

In the Clinical Note Review Dashboard, the evaluator can select clinical notes and begin the review process.

Note: Starting a clinical note review in the Clinical Note Review Dashboard requires the "Clinical Review Dashboard, Select Docs For Review" security right.

To start a clinical note review (Evaluator)

In the Clinical Note Review Dashboard window, select the Select Documents for Review tab if it is not already selected.

If applicable, do one of the following:

If the Password - Clinical Review Dashboard, Select Docs For Review dialog box appears, user verification has been assigned to the "Clinical Review Dashboard, Select Docs For Review" task. Do one of the following:

If your user account has been granted the "Clinical Review Dashboard, Select Docs For Review" security right, enter your credentials in the User ID and Password boxes, and then click OK.

If your user account has not been granted the "Clinical Review Dashboard, Select Docs For Review" security right, have a user with permission temporarily override this restriction so you can select clinical notes to review this one time. The overriding user enters his or her credentials in the User ID and Password boxes and then clicks OK.

If a message appears, informing you that you do not have rights to this operation, your user account has not been granted the "Clinical Review Dashboard, Select Docs For Review" security right. To have a user with permission temporarily override this restriction so you can select clinical notes to review this one time, do the following:

On the message, click Yes.

In the Password - Clinical Review Dashboard, Select Docs For Review dialog box, the overriding user enters his or her credentials in the User ID and Password boxes and then clicks OK.

The options on the Select Documents for Review tab are available.

For each clinical note in the data grid, the following information appears:

Document Status - The status of the review of the clinical note.

Visit Date - The visit date associated with the clinical note.

Medical Record Number - The patient’s chart number.

Document Type - Currently, the only type is Clinical Note.

Document ID# - The unique ID number of the clinical note.

Clinic - The clinic associated with the clinical note.

The following columns appear if the Show Patient Information checkbox is selected:

Patient Name - The patient’s first and last name (in a Last Name, First Name format).

Patient DOB - The patient’s birth date.

Note: To view the text of a clinical note, click the corresponding Click for Document link in the Launch Document column.

Change any of the following filters as needed:

Document Status - Leave Not Reviewed selected to start a review for clinical notes that have not been evaluated yet.

Document Type - Currently, Clinical Note is the only type.

Date Range - Clinical notes that were created within a specified date range will be included. The dates for the past three months are entered by default. To specify a different date range, do one of the following:

To include a specific creation date, enter or select the same date as both the starting and ending date of the range.

To include a range of creation dates, enter or select the starting and ending dates of the range.

To change the starting or ending date, do any of the following:

To enter a date, do any of the following:

To change the month, click the month portion of the date and then enter a month as a 1-digit or 2-digit number.

To change the day, click the day portion of the date and then enter a day as a 1-digit or 2-digit number.

To change the year, click the year portion of the date and then enter a year as a 2-digit or 4-digit number.

To select a date, do the following:

Click the corresponding calendar icon to view a month calendar.

Navigate to the correct month using the left and right arrows.

Note: To quickly change the year and month, click the month-year at the top of the calendar, navigate to the correct year using the left and right arrows, and then click the correct month.

Click the correct day.

Note: The date range cannot exceed 180 days.

Clinic - Clinical notes that are associated with any or specific clinics will be included. To specify clinics, do one of the following:

To include all clinics, leave the All check box selected.

To include specific clinics, do the following:

Click the search button .

The Clinic Selection dialog box appears.

Note: If your user account has been granted the "View All Patients" security right for the Central clinic, all clinics appear in the upper list; otherwise, the clinics for which your user account has been granted the "My Clinics, View" security right (in the "Practice/Clinic Setup" category) appear in the upper list; however, if your user account has not been granted that right to any clinic, only the clinic that you are currently logged in to appears in the upper list.

To search for a clinic by ID and/or practice name, do the following:

Click Search.

Enter the first character or more of a clinic ID and/or practice title in the ClinicID and/or Practice Title boxes, respectively.

Click Show Results to view a list of matching clinics in the upper list.

Note: To return to viewing the list of all the clinics, click Search, click Clear Search to clear the search criteria that you entered, and then click Show Results.

In the upper list, select one or more clinics.

Note: To select multiple clinics, click a clinic, and then, while pressing the Ctrl key, click the other desired clinics. To select a range of adjacent clinics, click the first clinic of the desired range of clinics, and then, while pressing the Shift key, click the last clinic of the desired range of clinics.

Click Add.

Repeat steps b-d as needed to add other clinics.

Note: To remove a clinic that you added, select that clinic in the lower list, and then click Remove.

Click OK.

ADA Code - Clinical notes that are associated with any or specific procedures will be included. To specify procedure codes, do one of the following:

To include all procedure codes, leave the All checkbox selected.

To include specific procedure codes, do the following:

Click the search button .

The Select ADA Codes dialog box appears.

Do one of the following:

To get a list of all procedure codes, do not enter any search criteria.

To search for procedure codes by category, abbreviated description, ADA code, and/or description, enter the first character or more of a procedure code category, abbreviated description, ADA code, and/or ADA description in the Category, User Code, ADA Code, and/or Description boxes, respectively.

Click Show Results to view a list of matching procedure codes.

Note: To clear the search criteria that you entered so you can search again, click Search, and then click Clear Search. Then, repeat steps b-c.

In the upper list, select one or more procedure codes.

Note: To select multiple procedure codes, click a procedure code, and then, while pressing the Ctrl key, click the other desired procedure codes. To select a range of adjacent procedure codes, click the first procedure code of the desired range of procedure codes, and then, while pressing the Shift key, click the last procedure code of the desired range of procedure codes.

Click Add.

Repeat steps b-e as needed to add other procedure codes.

Note: To remove a procedure code that you added, select that procedure code in the lower list, and then click Remove.

Click OK.

Evaluatee - The provider whose clinical notes you want to evaluate.

Notes:

You cannot select yourself.

The evaluatee of an unsigned clinical note is the provider who is assigned to that clinical note.

The evaluatee of a signed clinical note is the provider who signed that clinical note.

To select a provider, do the following:

Click the search button .

The Provider/Staff Selection dialog box appears.

Do one of the following:

To get a list of all providers and staff, do not enter any search criteria.

To search for providers by ID, first name, last name, and/or title, enter the first character or more of a provider ID, last name, first name, and/or title in the ProviderID, FirstName, LastName, and/or Title boxes, respectively.

Click Show Results to view a list of matching providers.

Note: To clear the search criteria that you entered so you can search again, click Search, and then click Clear Search. Then, repeat steps b-c.

Select the correct provider.

Click OK.

Chart Sampling Size - Do one of the following:

To show all clinical notes that meet the filter criteria, leave 100% selected.

To show a specific percentage of clinical notes (selected randomly) that meet the filter criteria, select Show 10 charts only (which is selected by default), Show 20 charts only, Show 5%, Show 10%, Show 20%, Show 30%, Show 40%, Show 50%, Show 60%, Show 70%, Show 80%, or Show 90%.

Other Options:

Include Unsigned Clinical Notes - To include signed and unsigned clinical notes, select this checkbox. With the checkbox clear, signed clinical notes are included, but unsigned clinical notes are not included.

Show Patient Information - To include additional columns for patient details in the data grid, select this check box.

Include 2nd Level Evaluator - To add a second-level evaluator, select this checkbox. With this checkbox selected, to select an evaluator, do the following:

Click the search button .

The Provider/Staff Selection dialog box appears.

Do one of the following:

To get a list of all providers and staff, do not enter any search criteria.

To search for providers and staff by ID, first name, last name, and/or title, enter the first character or more of a provider ID, last name, first name, and/or title in the ProviderID, FirstName, LastName, and/or Title boxes, respectively.

Click Show Results to view a list of matching providers.

Note: To clear the search criteria that you entered so you can search again, click Search, and then click Clear Search. Then, repeat steps b-c.

Select the correct provider.

Click OK.

Note: To return the filters to their default states and clear the data grid, click Clear Selections.

If you have changed any filters, or to make sure that you are viewing the most recent data, click the Refresh button .

The data grid is updated according to the specified criteria.

For each clinical note that you want to include in the review, select the checkbox in the Document To Be Included In Review column. You can select up to 10 clinical notes.

To start the review process for the selected clinical notes, click Start Review.

The Evaluation Report dialog box appears.

From the list at the top, select a clinical note review template to use the corresponding evaluation criteria.

Note: For information about managing clinical note review templates, see the topic about setting up clinical note review templates.

The first clinical note that pertains to this review is selected by default in the document list on the left.

 For each of the clinical notes that pertain to this review, do the following to evaluate the clinic note:

Review the note text in the box in the middle.

On the right, select Yes or No for all applicable evaluation criteria, and leave N/A selected for the criteria that are not applicable.

Do one of the following:

If the current clinical note is not the last clinical note that pertains to this review, click Next to navigate to the next clinical note.

The options for evaluating the next clinical note are available.

If the current clinical note is the last clinical note that pertains to this review, click Next to view the summary. Proceed to step 9.

Notes:

The Next button is available only if all the clinical notes have been evaluated.

A clinical note is considered to have been evaluated if there is at least one response to the evaluation criteria that is not N/A. Also, a green check mark appears next to the clinical note's document ID in the document list to indicate that the clinical note has been evaluated.

If you have evaluated all the clinical notes that pertain to this review, when you click Next after evaluating the last clinical note, the Summary page opens.

To add notes, click Evaluator Notes. In the Additional Notes dialog box, enter text, and then click OK.

If the evaluatee will not or cannot sign the review, do the following:

Under Evaluatee Signature, select the Refuse to sign checkbox.

From the list that appears, select the reason why the evaluatee will not sign:

Evaluatee is not present to sign the review.

Evaluatee no longer works at this clinic.

Evaluatee refuses to sign/acknowledge the evaluation.

Other reason(s). With this option selected, enter the reason in the box that appears.

Note: If the evaluatee will sign the review, complete and save the review without signing it. The second-level evaluator (if one is assigned) must also complete the review. Then, the evaluatee can sign the review.

Click Save.

Notes:

This button is available only if at least one clinical note has at least one response to the evaluation criteria that is not N/A. A green check mark must appear next to at least one clinical note's document ID in the document list, which indicates that the clinical note has been evaluated.

If you have not completed your review, you can resume it later.

The status of each clinical note that has not been evaluated is "Not Reviewed."

The status of each evaluated clinical note is "In Progress."
