You can remove a consent form from a case if the form does not have any signatures (patient and/or provider).
To remove a consent form
In Treatment Planner, do one of the following:
In the Treatment Plan Case Setup section on the Navigation panel, right-click the case that you want to delete a consent form from, and then click Consent Forms. The Dentrix Consent Forms dialog box appears.
With a case selected in the Treatment Plan Case Setup section on the Navigation panel, expand the Supporting Information section on the Navigation panel, and then scroll down to Informed Consent.
The consent forms that are attached to the case appear in the Form list.
Note: For each attached consent form, the date when it was signed (if applicable), its name, and who signed (patient and/or provider; if applicable) appear for your reference.
Select the correct form.
Click Remove Selected Form.
Note: This button is available only if the selected consent form does not have any signatures.
A confirmation message appears.
Click Yes.
