Printing questionnaires

You can print a questionnaire for a patient when he or she arrives for an appointment. The printed questionnaire has two pages: the first page one contains basic patient information, such as name, address, employer, insurance information, and referral sources; the second page contains the questions and may contain responses from the previous questionnaire (if one exists).

To provide the patient with a printout from which he or she can review personal and insurance information and medical and dental history

Notes:

You must create a Family File record for a new patient with an appointment before you can print a questionnaire.

Starting a new questionnaire for a patient requires the "Patient Questionnaire New" security right. Accessing and editing (optionally) the responses for a patient's questionnaire requires the "Patient Questionnaire Edit" security right. Viewing the responses for a patient's questionnaire requires the "Print Patient Questionnaire" security right.

Printing a questionnaire requires the "Print Patient Questionnaire" security right.

To print a questionnaire

In the Patient Questionnaire History dialog box, do one of the following:

To print a new questionnaire for the patient, do the following:

From the Select Questionnaire list, select a questionnaire.

Click New.

Note: You cannot add a questionnaire that has already been added today, but you can edit or view it.

If applicable, do one of the following:

If the Password - Patient Questionnaire New dialog box appears, user verification has been assigned to the "Patient Questionnaire New" task. Do one of the following:

If your user account has been granted the "Patient Questionnaire New" security right, enter your credentials in the User ID and Password boxes, and then click OK.

If your user account has not been granted the "Patient Questionnaire New" security right, have a user with permission temporarily override this restriction so you can start a new questionnaire for a patient this one time. The overriding user enters his or her credentials in the User ID and Password boxes and then clicks OK.

If a message appears, informing you that you do not have rights to this operation, your user account has not been granted the "Patient Questionnaire New" security right. To have a user with permission temporarily override this restriction so you can start a new questionnaire for a patient this one time, do the following:

On the message, click Yes.

In the Password - Patient Questionnaire New dialog box, the overriding user enters his or her credentials in the User ID and Password boxes and then clicks OK.

The Patient Questionnaire dialog box appears.

To edit (optionally) and then print an existing questionnaire for the patient, do the following:

Select a questionnaire.

Click Edit.

If applicable, do one of the following:

If the Password - Patient Questionnaire Edit dialog box appears, user verification has been assigned to the "Patient Questionnaire Edit" task. Do one of the following:

If your user account has been granted the "Patient Questionnaire Edit" security right, enter your credentials in the User ID and Password boxes, and then click OK.

If your user account has not been granted the "Patient Questionnaire Edit" security right, have a user with permission temporarily override this restriction so you can access and edit (optionally) the responses for a patient's questionnaire this one time. The overriding user enters his or her credentials in the User ID and Password boxes and then clicks OK.

If a message appears, informing you that you do not have rights to this operation, your user account has not been granted the "Patient Questionnaire Edit" security right. To have a user with permission temporarily override this restriction so you can access and edit (optionally) the responses for a patient's questionnaire this one time, do the following:

On the message, click Yes.

In the Password - Patient Questionnaire Edit dialog box, the overriding user enters his or her credentials in the User ID and Password boxes and then clicks OK.

The Patient Questionnaire dialog box appears.

To view and then print an existing questionnaire for the patient, select the correct questionnaire, do the following:

Select a questionnaire.

Click View.

If applicable, do one of the following:

If the Password - Print Patient Questionnaire dialog box appears, user verification has been assigned to the "Print Patient Questionnaire" task. Do one of the following:

If your user account has been granted the "Print Patient Questionnaire" security right, enter your credentials in the User ID and Password boxes, and then click OK.

If your user account has not been granted the "Print Patient Questionnaire" security right, have a user with permission temporarily override this restriction so you can view the responses for a patient's questionnaire this one time. The Print Setup dialog box appears.

Select a printer.

Click OK.

The patient summary, the questions, and any responses from the previous questionnaire (if one exists) are printed.

The patient corrects any inaccurate patient information, adds any missing patient information, answers any questions without responses, and updates previous responses to questions as needed.

To print the questions, do the following:

The Print Setup dialog box appears.

Select a printer.

Click OK.

The patient summary and the questions are printed.

The patient corrects any inaccurate patient information, adds any missing patient information, and answers all the questions.

Click Close.

Important information this report provides

Patient Information:

Full Questionnaire - The patient’s personal, referral, guarantor/spouse, employment, and insurance information stored in Family File.

Questions Only - The patient’s name, birth date, chart number, and Social Security Number stored in Family File.

Questions - Questions regarding medical/dental history and/or other custom questions, and the patient’s responses (if any).
