You can enter the responses for a patient's questionnaire into Dentrix Enterprise.
Note: Entering the responses for a patient's questionnaire requires the "Patient Questionnaire New" security right (to start a new questionnaire) and the "Patient Questionnaire Edit" security right (to enter responses).
To enter questionnaire responses
In the Patient Questionnaire History dialog box, from the Select Questionnaire list, select a questionnaire.
Click New.
Note: You cannot add a questionnaire that has already been added today, but you can edit it.
If applicable, do one of the following:
If the Password - Patient Questionnaire New dialog box appears, user verification has been assigned to the "Patient Questionnaire New" task. Do one of the following:
If your user account has been granted the "Patient Questionnaire New" security right, enter your credentials in the User ID and Password boxes, and then click OK.
If your user account has not been granted the "Patient Questionnaire New" security right, have a user with permission temporarily override this restriction so you can start a new questionnaire for a patient this one time. The overriding user enters his or her credentials in the User ID and Password boxes and then clicks OK.
If a message appears, informing you that you do not have rights to this operation, your user account has not been granted the "Patient Questionnaire New" security right. To have a user with permission temporarily override this restriction so you can start a new questionnaire for a patient this one time, do the following:
On the message, click Yes.
In the Password - Patient Questionnaire New dialog box, the overriding user enters his or her credentials in the User ID and Password boxes and then clicks OK.
The Patient Questionnaire dialog box appears.
By default, today's date appears in the Questionnaire Date box, but you can enter a different date (in a mm/dd/yyyy format). The specified date will appear on the applicable "Date" lines of a printed questionnaire.
Note: Dentrix Enterprise automatically adds slashes to dates when you enter six or eight numbers (mmddyy or mmddyyyy) and then press the Tab key.
Double-click the first question, or click Edit.
If applicable, do one of the following:
If the Password - Patient Questionnaire Edit dialog box appears, user verification has been assigned to the "Patient Questionnaire Edit" task. Do one of the following:
If your user account has been granted the "Patient Questionnaire Edit" security right, enter your credentials in the User ID and Password boxes, and then click OK.
If your user account has not been granted the "Patient Questionnaire Edit" security right, have a user with permission temporarily override this restriction so you can enter the responses for a patient's questionnaire this one time. The overriding user enters his or her credentials in the User ID and Password boxes and then clicks OK.

On the message, click Yes.
In the Password - Patient Questionnaire Edit dialog box, the overriding user enters his or her credentials in the User ID and Password boxes and then clicks OK.
A Patient Questionnaire Response dialog box appears.
Select or enter the correct response.
Click Next.
Repeat steps 7-8 for the other questions on the questionnaire.
Notes:
To review and/or change previous answers, click Back.
To not advance to the next question, click OK or Cancel.
When you return to the Patient Questionnaire dialog box, the responses appear on the right side of the list.
Click Close.
