Removing consent forms from cases

You can remove consent forms from a case if they are not signed or only have one signature. You cannot remove a consent form from a case if it has both signatures.

To remove a consent form

1.   From the Treatment Plan Case Setup panel, select the case that you want to delete a consent form from.

2.   From the Treatment Plan Case Setup panel toolbar, click the Consent Forms button.

The Dentrix Consent Forms dialog box appears.

3.   Select the consent form that you want to remove.

4.   To remove the form from the case, click Remove Selected Form, and then click Close.
