Editing consent forms

Dentrix consent forms are electronic and can be signed electronically. By default, these forms include Consent for Anesthesia, Consent for Treatment, and Decline Treatment Consent. Forms are stored in the Supporting Information panel. You can edit these forms and add new ones in the Settings panel of the Treatment Planner.

To edit a consent form

1.   To edit a consent form, click the Settings button, scroll to Setup Consent Forms, select a consent form, and then click Edit.

The Edit Consent Form dialog box appears.

 

2.   Edit the text of the consent form as necessary.

Note: Click the following buttons to edit the form's text: Cut, Copy, Paste, Insert Date Line, Check Spelling, and Display Font Size.

3.   If you do not want to include procedure codes on the consent forms, select Do not include Procedure Codes on Consent Form.

4.   To save the form, click Save, and then click Close.
