Deleting consent forms

You can delete consent forms that you created if you no longer need them.

To delete a consent form

1.   Click the Settings button, scroll to Setup Consent Forms, and then select the consent form that you want to delete.

2.   Click Delete.

A confirmation message appears.

3.   To delete the form, click Yes.
