Entering patient responses

You can open questionnaire forms directly from the Questionnaires module so that your patients can enter their responses electronically in your office. This feature is useful if you have a kiosk, operatory computer, or other computer that patients can use to enter responses, or if patients want to dictate responses to you as you enter them.

Note: If you are going to allow patients to enter their own response information on a computer in your office, it is important to have the appropriate passwords set up to protect the other areas of your software.

To enter patient responses

1.   In the Questionnaires module, select a patient.

The Questionnaires List pane changes to include information specific to the selected patient.

2.   In the Edit menu, click Enter New Responses.

The Select Questionnaire Form dialog box appears.

Any default or custom form that has not been signed electronically is listed in the Select Questionnaire Form dialog box, regardless of whether the patient has given responses for the form.

3.   Select the questionnaire form that you want and click OK.

The selected form appears.

4.   Enter the response information in the form.

To navigate the pages, click the arrows at the top or bottom of the form.

Note: When patients fill out responses for several forms at once, they only need to fill out their patient information once. The responses in the patient information fields of subsequent forms default to the information that was entered in the previous form.

5.   When you have entered all your responses in the form, click Submit to save your responses.

The responses are saved, and the form is moved from the Needs Responses tab to the With Responses tab.

Note: The Submit button is disabled until the last page of the form.
