
The Questionnaires module includes the Questionnaires toolbar, the Dentrix modules toolbar, and the Patient Information toolbar.

Click to open the Update Patient Information dialog box from which you can update the patient’s record in the Family File to match new Questionnaire information. Note: This button is only active when a form that has responses is selected in the Questionnaires list box. |

Click to edit the responses for the selected questionnaire. |
