Inactivating questionnaires and consent forms

You can inactivate a questionnaire or a consent form that you no longer want to use. Inactivating a form does not affect the records of patients who have completed the form previously. When you inactivate a form, it will no longer be available to patients.

Notes:

  • Each questionnaire, including its activation status, is organization specific.

  • Each consent form, including its activation status, is location specific.

To inactivate a patient form

  1. If you are not already viewing the correct location, select it on the Location menu.

  2. On the Settings menu, under Location, click (or tap) Patient Forms.

    The Patient Forms page opens.

  3. Under Questionnaires or Consent Forms, select an active form (the name will be in black text to indicate that it is active).

    The options for the form become available.

  4. Set the Active switch to Off.

  5. Click (or tap) Save.
