Enabling Dentrix Ascend Forms

If you are ready to convert your legacy forms and start using the new forms, you can enable the new patient forms features.

Important: Before you switch to the new patient forms, you should understand the following:

  • Your legacy forms (questionnaires and consent forms) are converted to the new forms solution. For those of you who already started building new forms (or even started having patients complete the new forms) prior to this phase of the release, the legacy forms are available as templates that you can use to create new forms.

  • Active forms that are converted have a "Published" status. Inactive forms that are converted have an "Unpublished" status.

  • With the legacy forms system, the online portal has a "Terms of Use" that patients have to acknowledge to use the site, but that form does not exist with the new online portal. If you want to provide a similar consent form with the new portal, you have to create a form and publish it in the new forms system.

  • Until the new patient forms system is fully released, you may see duplicate forms in a patient's record. For instance, if an existing patient has completed or is due to complete a "Dental History" form, you will see a "Dental History" form that corresponds to the legacy system and a "Dental History" form (converted from the legacy system) that corresponds to the new system.

  • Some workflows are different and may require time for adjustment, including the following:

    • A copy of a completed form is saved in the patient's Document Manager as a .pdf file.

    • The demographics and medical conditions/allergies entered on a completed form can be updated in the patient's record automatically.

  • Several features are not available yet, including the following:

    • Multi-lingual forms. If you use bilingual consent forms in the legacy forms solution, because only the English forms are converted to the new forms solution, save a copy of the non-English text for your consent forms before upgrading so you can reuse that text once bilingual forms are available.

    • Provider editing/signing of forms.

    • Push-to-kiosk is no longer available, but a QR code is available for scanning with an office-owned device.

Caution:Enabling the new patient forms for a location CANNOT BE UNDONE. Make sure that you understand the repercussions of enabling them before proceeding.

To enable the new patient forms features for a location

  1. If you are not already viewing the correct location, select it on the Location menu.

  2. On the Settings menu, under Location, select Patient Forms.

  3. On the Patient Forms page, click (or tap) Try New Forms.

  4. In the Try New Forms dialog box, click (or tap) Upgrade Now.

Note: After you enable the new patient forms features, to manage forms, you select the Form Builder option on the Settings menu.
