Viewing Patient Responses in the Questionnaires Module
When form responses are submitted, they can be viewed in the Dentrix
Questionnaires module. From here, you can update the patient's Family
File or Health History based on their answers to the forms.

Additional Information
When selecting Update Patient Information for a new patient, the
Select Patient window will open. Select the matching New Patient
appointment, and Dentrix will create a Family File for them. Then use
the Update Patient Information process to populate the information
into the patient's file!
If an existing patient fills out forms using the New Patient Journey
option, you can still connect their response to their patient record.
When you click Update Patient Information, the Select Patient window
will open. Simply select the patient to tie these responses to that
patient.
