The Questionnaires window

The Questionnaires window consists of the menu bar, toolbars, Questionnaires List pane, and Questionnaire View pane. The Questionnaires List pane consists of the following five columns:

·        Form Name – Name of the form or questionnaire.

·        Response Date – Date a patient responded to the form or questionnaire.

·        Expiration Date – Indicates when or if the form or questionnaire expires.

·        Update Patient Info – Indicates whether a patient's information needs to be updated.

·        Signed – Indicates whether a patient signed a form or questionnaire.

The List pane is also divided into a Completed section and a No Responses section, so that you can see at a glance which questionnaires patients have responded to and which they have not.
