As long as a response has not been signed, you can delete patient or provider/staff responses that have been entered into a questionnaire form. If any signatures are attached to a questionnaire form, you cannot delete the form without first deleting the questionnaire signatures attached to it.
Note: Deleting responses only clears the information entered, not the form itself.
