In Dentrix, you can create templates based on the expected periodontal conditions of certain groups of people (healthy adult, tobacco chewer, or tobacco smoker for example). You can also rename or delete any exam template that you create.
To rename or delete an exam information template
1. Select a patient, and then click the Exam Information tab in the Perio panel.
The Exam Information section appears.
2. Click
Template Setup 
.
The Exam Information Template Setup dialog box appears.
3. Click Manage Templates.
The Manage Saved Templates dialog box appears.
4. From the list, select the template that you want to rename or delete, and do one of the following:
· To rename the template, click Rename, and type the template's new name.
· To delete the template, click Delete.
5. To save your changes, click Close.
