Health History lists any Questionnaires forms your patients have submitted. You can view a submitted form by double-clicking it, or you can open the Questionnaires module by clicking the Questionnaires button under Health History Questionnaire Forms.
To view a Health History Questionnaire form
1. In any module except the Office Journal and eDex, click File, point to Switch To, and then click Health History.
The Health History window appears.
2. To open Questionnaires, double-click a form or click the Questionnaires button.
The Questionnaires module opens.
