Since the Patient Chart is a clinical record of patient care, it must be completely accurate. To maintain an accurate clinical record, you can correct errors in the Patient Chart to reflect the actual care given by invalidating procedures that are in history. Invalidating a procedure removes it from the Patient Chart, while leaving it on the Ledger.
You can also invalidate conditions and diagnoses instead of deleting them to preserve their history and maintain an accurate clinical record of a patient's care. Invalidating a condition or a diagnosis does not remove it from the Patient Chart.
To invalidate a condition or diagnosis
1. Select a patient.
2. From the Progress Notes panel, double-click the condition or diagnosis that you want to invalidate.
The Change Condition/Diagnosis dialog box appears.
3. If necessary, type a new date.
4. Select Invalidate Condition/Diagnosis.
5. Type a note, and then click OK.
The condition or diagnosis is invalidated, and an asterisk is placed in the Description column in the Progress Notes panel.
